Assistance in coordinating life stages, transitions & supports.This program includes short and long-term supports that focus on strengthening the participant’s ability to coordinate their supports, and to assist them to live at home and participate in their community. Our goal is to empower participants living with disabilities to build connections and engage with their community in various meaningful ways. These include:
Assistance with travel or transport arrangements
Our service provides travelling to and from desired places to attend appointments or activities away from home. We value the significance of reliable source of transport and comfort for participants who require assistance with transportation to and from social visits, appointments, shopping, educational and recreational facilities, or work.
Assistance with daily life tasks in a group or shared living arrangement
Our program incorporates assistance with and/or supervising tasks of daily life in a shared living environment, which is either temporary or ongoing, with a focus on developing the skills of everyone to live as autonomously as possible. This includes short-term accommodation and assistance (e.g., respite care). We provide supports for participants with personal care in disability accommodation and shared living arrangements.
Our supports focus on promoting individuals’ capacity for skill development and self-determination relating to the unique and various needs. These include supervision or assistance with personal care and hygiene; showering and/or toileting; dressing; mobility; appointment assistance; domestic task; activities of daily living; short term accommodation and supported independent living to assist clients to achieve their desired level of independence. Group or Centre based activities
Our group and centre-based activities help participants with opportunities to engage in enjoyable social and recreational activities, find their interests, and develop new skills. Our aim is to provide a fun and inclusive environment for clients’ participation, socialising, interaction, and building relationships. Our goal is to encourage participants to explore their passion, acquire new skills, meet new friends in a supportive environment while enjoy themselves. Our team believe that group and centre-based activities participation promotes sense of wellbeing and community belonging.

Assistance with daily personal activities Assisting with and/or supervising personal tasks focuses on the development of daily personal activities of daily life to enable participants to live as autonomously as possible. Our goal is to provide a supportive and engaging environment to develop skills and empower participants to build confidence, increase independence, and achieve their goals. These individual supports can be provided in a range of environments, including but not limited to, the participant’s own home.
Participation in community, social and civic activities
Assisting participants to participate actively in community, social and civic activities; includes supporting participants during these activities and developing participants’ ability to partake in these activities. Carers on wheels values the importance of community engagements and social connections for participants. We assist participants in meeting new people with similar interest and joining in social and community activities. We support individuals to build new connections and enhance their community engagement to empower them to achieve their aspirations.
Development of daily life skills
This service focuses on training and development of activities undertaken by the participant to increase their ability to live independently, including supports that will enhance the ability of the participant to travel and use public transport independently. Our team is committed to assist clients to develop skills and independence in managing activities of daily living and own personal care. Our services include assisting with personal care, showering, dressing, toileting, mobility, meal preparation and domestic tasks.
Household Tasks
Our team is committed to assist participants with disabilities with essential daily household tasks that the participant is not able to undertake to maintain their homes. These are tailored according to individual unique needs. Our staff assist with a wide range of tasks, including such as house or yard cleaning or maintenance, meal preparation and delivery, shopping, laundry, and errands. We provide flexibility for individualised care to ensure our participants enjoy living in clean and comfortable homes.
Community Nursing Care
This is one of our most popular services with guaranteed satisfaction that provides the highest level of excellence and quality care. With this service, we ensure all details are simple, seamless, and handled in a timely manner. We provide specialist care for participants who have high care needs requiring a high level of skill, and our support workers are trained to respond to the participant’s complex needs.
Our experienced professionals are dedicated to assist participants with various and unique medical needs. Our goal is to assist participants to improve quality of life, attend to their medical needs through the provision of ongoing evidence-based health care, education, and client centred care planning in collaboration with participants and families.
